Losing a passport is probably one of the things anyone of us won’t want to experience, specially if you’re a traveler by heart or a normal person scheduled to travel, but alas it happened to me! Indeed, I consider this experience as one of the highlights of my year 2012, probably not a happy highlight but I try to find a positive side in this seemingly unfortunate event, and that is to share with you all the things i learned and information you should know in the event that you’ll find yourself in the same fate as I had.
But before you go along reading my blog, let me give you a top view of what to know in this short excerpt of mine

So ok, if you still find this blog interesting having mentioned the above items, let me then discuss further Item #1.
How many days it will take you to secure your new Philippine passport?
It will take you roughly 3 weeks to a month to secure a replacement for your lost passport. 15 days of which will be dedicated to the verification process of your Police Report and Affidavit of Loss (to be discussed below). Passport processing can then be of two options: Regular Processing (15 working days) or Express Processing (7 working days). I took the Express processing so that’s 15 days verification + 7 working days processing, so that’s about 21 days for me, but alas once again my application date fell in between the Holyweek period so additional 5 days waiting for me.
The first things you should do in the event that you lost your Philippine Passport
Two things, file an Affidavit of Loss and Police report immediately after losing your passport. You can get an Affidavit of Loss from any attorney-at-law nearest you, in here you will state how you lost your passport, when, where, and other circumstantial event in relation to the incident of losing it. Get 2 copies of this, I got mine for Php20o both. And make sure you have these 2 copies notarized. I was able to have mine notarized at Php50 each. Now, the Police Report, go to the police station covering the area where your passport was lost. I lost mine in Herrera street, Makati, therefore the covering police station is the Makati Police Station, apparently they have a head quarter near the van terminal where I get my ride going home. So i dropped by the next day to the police station to ask for a Police Report. This will cost you Php50. I had to pay mine thou in Makati Central Police Station in Yakal street, i looked for the cashier window, paid and took the receipt and went back to the station where I filed the police report. You will get this same day.
You might ask, do I need to file a Police Report even if mine wasn’t lost due to crime, like snatching, robbery, etc? what if i just lost it by accident or just misplaced it? the answer is YES, no matter how or what manner you lost your Philippine Passport you will still need to file a Police Report.

Documents and Processes you will undergo when you lost your Philippine Passport
Aside from the Notarized Affidavit of Loss (2 copies) and Police Report (2 copies) you also need to prepare the ff:
1. Original NSO Birth Certificate
2. Government ID (note: they are particular in this, they don’t accept Postal ID, and new Company ID). Better if you can bring your TIN, SSS, Alumni ID, or if you don’t have this, they will ask for other old documents such as you Cert of Baptism, Old NBI clearance, old police clearance or even your college yearbook.
Make sure you have all these photocopied, aside from the hassle of leaving the line if you will do it in DFA the price is an instant highway robbery for you P3.00 per copy! imagine that!
Process
Step 1: Secure an appointment first by logging in DFA’s electronic appointment sytem: http://www.passport.com.ph/set-appointment
Note: they strictly implement the “No appointment No Entry” Policy. so better set appointment first.
Step 2: Print your appointment schedule then come on time on the stated date. Don’t be frustrated thou, don’t expect much on this appointment, you will just submit your Affidavit of Loss and Police report on this stage (bring your other docu as well in case). They will stamp a date of return on your Affidavit of Loss copy. You then need to come back for the 2nd time in DFA to start your passport processing. and the come back for the third time for pick-up. See my actual sched:
(Day 1) March 15 – Submission of Affidavit of Loss and Police report *subject for 15 days verification
(Day 2) March 30 – Passport Processing. I opted for the express processing (7 working days, but since it will be Holy week i have additional waiting days)
(Day 3) April 13 – Pick-up my Passport! Hurray!!!!
Note: you can choose to have your passport delivered. Just look for the 2GO booth inside DFA and pay for the delivery fee.

So now let me share to you the expenses due to this event

yeah that’s still true. but in other countries like Malaysia you can get a passport replacement same day, just like a vendo machine. you just input your data, then pay the machine then it will release your new passport same day. 🙂

Sana ganyan din ang Philippines like Malaysia you can get a replacement of your passport on the same day. Nakaka stress ang DFA specially when you lost your passport and you are scheduled to travel next week. Sana may consideration sila if naka booked na lahat.

hi meandmychucks, ask ko lang po kung anu po dapat gawin sa lost passport at expired narin po dis sept. naapply ko na sya before lost passport kumpleto pa po ako ng requirements kaso nga lang po nung magfefee ako last 2011 hinanapan ako ng school id ee wala akong dala. ngayun ko lang nahanap ung college id ko.. pede ko paba icontinue na iaapply ung naapply ko last 2011? at married na rin ako anu po ba mga requirements sa lost passport na maeexpire na sa sept.8 and gusto ko narin ipachanges stat. thx

I suggest you just apply a new passport and declare it as lost passport. Bring as much documents as you proving your identity, kakailanganin nila yun for decalration of lost passport.
Then from there you can change you maiden name and status.

You will need to go to DFA directly and tell them you can’t complete the online appointment because you dont have your old passport number. The guards will let you in when you tell this. Then from there, they will advise you how to move forward.

You can only apply for a SCHEDULE of your passport application. As of the moment DFA doesnt allow online passport application, you still need to go to DFA to personally apply it but you have to have a proof that you scheduled onlline. you may refer to this link as i posted in my blog.

You have to have that before you can set an appointment. try to see if you have written your passport number else where. but if you really cant I suggest go to DFA straight and tell them your situation. it’s always important to have a back up copy of your documents.

Madali lang kumuha ng police report sa pilipinas. you just have to explain whay happened and they will write the report for you. no need for supporting documents when applying for a police report

i just experienced the same, nadukutan ako dito sa Jakarta, Indonesia and my passport is inside my stolen wallet. Dito naman the most important thing is you have to file for a police report, buti na nga lang i was able to have a xerox copy of my lost passport….I went to the Philippine Embassy here, fill the application form as well as the affidavit of loss….Medyo mahal nga lang, Php 6,500.- and i added another Php 500 for the delivery fee. I was able to ask also for any consideration sa charges because i am a victim of pick pocket, i lost my money, atm and credit cards, but still ganun pa din ang siningil sa akin……..:(

I’m not sure pano ang police report dyan sa Malaysia. and I’m not sure din pano ka makakabalik ng pinas kung wala kang passport pabalik. I suggest you go to Philippined embassy dyan sa Malaysia so you can ask for details.

Yearbook is just supporting, you still need to bring other valid IDs. I used my Alumni ID, you can apply for that in your school as well.

There’s a possibility, but you need to write a letter to DFA about your situation and submit supporting documents na kailangan mo tlga makaalis by Jan 7. Normally pag employment ang case they allow, but you have to check with them training related. just bring all supporting documents with you.

may format na ang police headquarters mismo. you can get it from them. sabihin mo lang you need it to apply for lost passport.

you will need your company to write to POEA that you’re currently employed by them. POEA then will review this letter and they will process your OEC. if you still have OEC naman, you can use it as well.
Tell you company to email POEA at balikmanggagawa@poea.gov.ph. They call it the Visa/Guarantee Letter, basically it just state your company’s name and they are acknowledging that you’re still working for them.
Also, go to the main office of POEA, the one in Ortigas, don’t go to satellite offices, they will just tell you to go to the main office.

hello Miss Chucks! i just want to clarify something regarding visa stamps, i just finished my contract abroad,and now I’m back here in the philippines, recently i had my passport renewed,unfortunately i got rid of my old passport which has my visa stamp,now i’m applying for work abroad again,though i’ve got my Certificate of Employment with me to show for my agency,still my concern is would my agency be able to trace or track my travel record now that i dont have my old passport which contains my visa stamp?thank you..

I am not 100% sure, but you might need to re-apply your visa. When i lost my passport, I had my Japan visa in it, I went back to the processing agency after i got my new passport and asked them if I can still enter Japan even thou my visa was in my lost passport. Unfortunately they said, I have to have that visa in my current passport, and for me to have that, I need to re-apply and do the entire japan visa process once again. 😦

It’s possible that they will use your passport for other identity, but the latest one, yung may chip is mahirap na i-tamper. But I highly suggest i-report mo na rin sa DFA to make sure di ka magkaproblema in the future. you will need to prove your identity with all the supporting documents na ikaw nga yung taong yun.

Di lang ako sure if you can ask it from DFA. I tried the same thing when I lost my passport just in case they need my passport number. but then they didnt asked me for it, your case is different thou.
call DFA on this or tell your mother to o to the Philippine embassy there in UAE to consult.

Legally po akong tourist pumasok dito sa malaysia, with my round trip ticket and extra fund,, extended for 1 month social visit- i’ve been extend my passport once at brunie- but twicely wala na po hanggang ngayon expired na more than 1 year,,,- luckily nakauwi na po ako dito pinas with ilegal- sumama lng po ako sa legal speadboat pauwi ,,,

This is a tricky situation, because if you exited Malaysia illegally they will see that, because you don’t have your exit stamp from Malaysia.

I think it will be best to go to Philippine embassy there in Riyadh, because even thou you’re already resigned there, you still need to present your passport to the immigration when you exit Riyadh and when you enter Philippines. I am sure they will look for that.

not quite sure regarding your question. But what I know is you need to establish the identity of a person if you are filing for a lost passport. This is for them to make sure the person exist and securing a passport won’t be use for fraudulent acts. Better go to DFA’s website to check on this. they have a help desk where you can email your concern.

I think you will still need to declare it as lost passport, because if you declare it as expired, they will ask you to proceed that expire passport and if you won’t be able to produce it they will ask you to follow the process of applying for lost passport.
If you don’t remember your passport number, just do a walk-in and tell the guards that you cannot finish the online appointment because you don’t remember your old passport number anymore.. they will let you in.

I don’t think there will be a problem, the police report and AOL is just a record they keep so in case may mag-file under your same identity you are covered.

My question is, there is no time indicated on the return date and I forgot to ask the officer. What time did you come back? Do I need to set another appointment?

Normally, they have the date indicated on your return. But if you missed this part, the average return day is 15. You may come back 15 working days after your submission.

If you don’t remember your passport number, just do a walk-in and tell the guards that you cannot finish the online appointment because you don’t remember your old passport number anymore.. they will let you in.

my mom lost her passport in Riyadh…..ilang beses n kme pabalik balik ng dfa….still ang laging cnsv ng embassy sa riyadh wala p rin daw……khit ang sv sa phil embassy eh andun n ung record ipaverify n lng……pero laging cnsv wala p rin daw…….

I suggest ask our embassy in Riyadh what she can do to apply for a new passport. I am not sure how the ruling is kapag sa ibang bansa (not from your originating country) nawala ang passport.
You may inquire from Philippine embassy there in Riyadh.

Unfortunately, no. the stamps and visa she had in her passport won’t be retrieve. She will have a totally new passport.

I processed my new passport sa main office. Better if you do it as well.
DFA won’t disclose their verification procedure. Pababalikin ka na lang nila after 15 days then dun mo malalaman if you’re allowed to process a new passport. If you don’t have questionable record naman and you can prove na lost passport siya, then no need to worry.

Hi,i also lost my passport. I already got my police report and affidavit of loss,but my NSO birth certificate will be available on July 31 for some reasons. I am planning to set an appointment on July 18,so im going to DFA without my NSO birth certificate. Will that work kasi sabi mo papabalikin ako after 15 days pa. Kukunin lang ba nila yung police report at affidavit? Do i still need an appointment kahit papabalikin pa naman ako. Thanks so much. This blog is a big help.

Unfortunately, you have to have that NSO authenticated birth certificate. They won’t accept it if it’s not the NSO one. I suggest make a new appointment and time it when you already have your NSO BC.

apply ka as lost passport. kasi pag declare mo na expire passport hahanapin pa rin yung luma. since wala na sa yo yun you have to decalre it as lost and follow all the process I mentioned sa blog na ito.

Ganun na nga. pag nakuha mo na ang bagong passport mo, wala na duon and mga dati mong stamp, bago ang lahat ng pages ny passport mo and walang paraan para mastampan ka ulit.

If passport is lost/stolen in the middle of your trip, then it is double the hassle.
I just recently returned from an eventful European Trip. Bag containing my passport and wallet was stolen in Budapest. If you ever lose your passport there, better visit the Embassy in Vienna and not the Budapest PE. The PE in Budapest was not helpful!!! I am based in the UAE and their immediate reply to me was “Go back to Manila!” which will cost me more or less 2000USD. I was even the one who suggested to them “maybe we can expedite the renewal of the passport and have it couriered here in Budapest?”. They didn’t know what they were doing. When the ambassador arrived and asked if the police report was done, they all panicked and said “No… but………” (Obviously that’s the 1st thing to do!!!!)
I ended up getting a “green passport” (the old machine readable one) in Vienna.
I am normally the one who let this kind of things pass, but if it could help another soul in the process, then better vent here! LOL. I hate to say this but those were one of those days when I wished I was not holding a Filipino Passport….

Thank you for this helpful info, Indeed that’s sad, losing your passport in the middle of your travel (which is suppose to be stress-free) and our PE in Budapest not being helpful. However, on the brighter side, there’s our PE in Vienna. Guess you had more reason to stay longer… that couldn’t be that bad right? 😉

Try calling your company or go to UAE embassy there sa Philippines. They can help you, either the grant you the visa or they issue you a letter to pass the immigration, however I think you will need to inform your company about this for they will need to prove that you work for them, That is between your company and the UAE embassy.

If you have pending application with your passport, I suggest don’t declare it as lost. Either your current application (whatever it is) might get affected.
Plus if you file for it as lost sa Police then in the future you will use this “declared lost” passport baka magkaproblema ka.

Did you try to go back to DFA and ask what happened to your application? or if you had it delivered to your house ask the courier who did it for you (I’m assuming it’s 2go?) ask them and they can track where your passport is.

I suggest you go to the embassy directly instead of calling. bring your documents and the stamp of your passport release. I am not quite sure how it works back there in Hongkong.

I just want to ask about the expenses. When did you pay the lost passport fee and the processing fee? Did you pay everything at once on your first, second, or third visit to DFA Office? Or did you pay separate fees on different dates?

I’m just confused though as you mentioned you still had to get a police report but when I checked the website, it says there’s no need for a police report when the lost passport is no longer valid:
“GENERAL REQUIREMENTS:
Personal appearance
Confirmed appointment
No need to submit a passport size photo
Submit same documents required for first time passport applicants.
ADDITIONAL REQUIREMENTS:
In case passport was lost and cannot be found:
Submit Notarized Affidavit of Loss (with detailed explanation on when, where and how passport got lost).
If the lost passport is still valid, submit Police Report.
Photocopy of first page of lost passport (if available)”

yes no need for police report if your passport is already expired. as for my case my passport is not yet expired and has 2 more years validity so I had to file a police report.

Hi thanks for your blog and all other comments/tips given by the others – very helpful indeed. I too lost my passport weeks ago and went through the process including the online appointment. I got mine in Alimall QC and was there this morning only to be told that they can’t help me and that lost passports are ONLY processed in DFA Aseana. Anyway good thing they have a feedback form. I suggest they program their system in such a way that lost passport applications get directed only to DFA Aseana and nowhere else. It will really save a lot of precious time. Thanks again for this blog – learned a lot. Like you, i felt thankful na lang i had to experience losing my passport. It was my first time to get a police report – you know – i had to sit just 2 feet away from the jail (yes, full packed) and saw in detail that place where they take the mug shots. Well it was quite an experience. But the hassle is real. My 3 kids now can’t get theirs renewed until i get my new one. Oh well…:))

Hi, regarding police report. Do I have to get it from the head quarters or just the police station covering the area where I lost my passport?
I went to the police station, near us, where I also lost the passport. the front desk officer said that ‘MAKIUSAP ka sa investigator, KUNG GAGAWIN dahil sa head quarters talaga ginagawa ang police report. ‘
I’m confused, why do I have to begged if it’s their job, and they want me to wait for the investigator but there’s no assurance that they will do it.
